The following are guidelines for submitting your anniversary announcement to The Times. We publish every 5th anniversary, starting with 25 years. Information, especially names, must be typed or printed clearly. All forms must include a daytime phone number. Fill out the form (front & back) including first and last names (i.e. Jane Smith, not Mrs. Mike Smith). The completed form should be submitted at least two weeks prior to publication.

We will publish one photo. The photo should be good quality black & white or color with no guarantees of good reproduction in the newspaper. Photos will be returned by mail only if a self-addressed stamped envelope is included with the completed form or the photo can be picked up after publication.

The completed form should be submitted during the same month of the anniversary date and at least two weeks prior to publication.
